Documentation:
Documentation supporting requests for academic accommodations must be completed by a licensed independent professional, i.e., medical doctor, physician, psychologist, psychiatrist, etc. The documentation submitted to the office of Disability Services for Students is intended to help establish the student's disability/condition and related impact in the educational setting (see Documentation Requirements).
Academic Accommodations are based on;
- identification of the nature and extent of the disability provided by a qualified licensed professional
- associated functional limitations as related to the academic environment
- a description of the current course of treatment including medical side effects and prognosis for the disability
- recommendation for reasonable accommodations
The office of Disability Services for Students provides students with a variety of verification forms: Health-Related Packet, Mental Health Disability Packet, ADHD Packet, Autism Disorders Packet--that can be downloaded and completed by the student's provider if other forms of documentation are not available or sufficient.
Once the application is completed and submitted:
Once a student’s application and supporting documentation are submitted to the office of Disability Services for Students, the applicant will be contacted by the DSS staff to schedule an Initial Meeting. During the Initial Meeting, the student and Disability Coordinator will review the student’s application and documentation to determine appropriate academic accommodations. This process may take up to five business days for students who are currently enrolled in classes.
At the postsecondary education level, students are solely responsible for decisions regarding their accommodation needs. Parents may attend meetings with the student and the Disability Coordinator at the student’s discretion. Students will also be provided with instructions regarding faculty notification and their responsibilities after the initial meeting, including the request for academic accommodations for classes each semester.
