Camps and Programs for Minors (CPMs)
The Office of Continuing Education oversees all camps and programs for minors (CPMs) on campus. Below, Dedicated Program Directors can find documentation required and rules regarding CPMs. Please adhere to TAMIU Rule 24.01.06.L1, Camps and Programs for Minors in order for your CPMs to be compliant. CPM Training is provided by the Office of Continuing Education in the Fall and Spring semesters. Individual trainings can also be scheduled. For dates and times or to further discuss your CPM, contact the Office of Continuing Education at 956.326.3068.
All CPMs must begin submitting paperwork 3 months prior to the start of the CPM.
FORMS FOR ALL CPMs
**Must be submitted 2 months prior to the start of the CPM:
Upon review of CPM Application, a Determination Form will be sent to Dedicated Program Director with further instructions.
FORMS FOR CPMs THAT FALL UNDER RULE
**STAFF FORMS: Must be submitted 3 weeks prior to the start of the CPM:
- Waiver, Indemnification, and Medical Treatment Authorization Form (per participant)
- Model Release Form for Adults
- Child Protection Training for Non-Employees (Instructions Sheet)
- CPM Staff Code of Conduct
- Employee Roster
- Department of State Health Services Training Certification Form
- CPM Acknowledgment Form (Acknowledges having received CPM training and emergency plan. Please also submit sign-in sheet of the CPM training given to staff and volunteers with date of the CPM training.)
- Incident/Injury Report Form (Submitted only in the event of an incident or accident)
**PARTICIPANT FORMS: Must be submitted no later than THE first day of the CPM:
- Waiver, Indemnification, and Medical Treatment Authorization Form (per participant)
- Model Release Form for Minors (per participant)
- Medical Information & Release Form
- Participant Roster (Submitted in any legible format on the first day and last day of CPM)
FORMS FOR CPMs THAT DO NOT FALL UNDER RULE
**PARTICIPANT FORMS: Must be submitted no later than THE first day of the CPM:
- Waiver, Indemnification, and Medical Treatment Authorization Form (per participant)
- Model Release Form for Minors (per participant)
- Employee Roster
- Participant Roster (Submitted in any legible format on the first day and last day of CPM)
